in blender, like this :
Add a plane next to the camera, and make sure its normal heads exactly in the same direction. You may want to parent it to the camera. Then put the emit value to the max (1.0). And you're done.

I hate on camera flash
I like hot shoe flashes, but tripod mounted works best (doh).
Bouncing off the ceiling at a slight angle left or right gives a very neat effect for close up stuff, background falls off very quickly (see the top only guy).
I'm thinking it wouldn't be too hard in blender to make the mesh emitter be parented to a couple bones so you could rotate it about the top of the camera to front, ceiling, walls etc.
