That's probably caused by an old Maxigo version that is not supported in Max 2013.
You can get the latest Maxigo from here:

You should also delete the previous incorrect Maxigo installation.

You need to disable gamma correction if you use Maxigo.
I don't know about the IndigoMax.

Those are emitters with alpha maps.
You just need to have good photos of fire. And yes, don't use step blend.

I'd recommend 448 core version of 560 Ti. Faster than regular 560 Ti.
