The issue has to do with your curvature threshold setting. Areas with less curvature are getting subdivided less than other areas which results in smoothing artefacts. You should set it to 0.0 in your case. The default setting of 0.1 is usually good for situations where you are subdividing for the purpose of displacement mapping. However, if you are subdividing and smoothing in order to get a super-smooth mesh, you should set:
1. curvature threshold to 0.0
2. Turn view dependent off
Also, six subdivisions is major overkill on this mesh. 3 should be plenty.
You will find it extremely helpful to change the material to diffuse, and then enable 'random triangle colors' in the Indigo GUI. This will let you see the effects of your subdivision settings.
